If I spot what appears to be a pair of duplicates which are listed in the same set, should I not bother reporting it?

Correct. Placing the still into set means that a moderator has already made inspection of suspected duplicity and decided that keeping it is in benefit of MovieStillsDB users.

Duplicities that were accepted unconsciously are an issue. MovieStillsDB has build in tool for reporting duplicities to moderators, but it does not always work. And when a gallery has hundreds of stills, it is very time consuming for moderators to check whether the still is already there or not, so this torturous process is skipped most of time.
